What Is Lipedema?

Lipedema is a condition characterized by an abnormal accumulation of subcutaneous fatty and connective tissue, most commonly involving the:

  • Knees
  • Lower legs
  • Upper arms

Unlike typical weight gain, lipedema usually occurs symmetrically and can create a noticeable disproportion between the upper and lower body.

Patients may also experience pain, tenderness, a feeling of heaviness, easy bruising or nodular areas beneath the skin.

One classic feature is that the enlargement of the legs may stop around the ankles while the feet remain relatively unaffected, sometimes creating a visible “cuff” above the ankle.

Lipedema vs. Obesity: What’s the Difference?

Lipedema and obesity are not the same condition — although a patient can have both.

With obesity, excess fat typically accumulates throughout the body and generally decreases as a patient loses weight.

With lipedema, the abnormal fatty tissue tends to accumulate disproportionately in specific areas, particularly the legs and hips. Patients may lose significant weight from their abdomen, chest and face while their legs remain disproportionately enlarged.

This is why some patients become frustrated after substantial weight loss: the number on the scale may improve dramatically, but the disproportion associated with lipedema remains.

Recognizing that difference is an important part of developing the right treatment plan.

Can GLP-1 Medications Treat Lipedema?

GLP-1 medications can be used as an adjunct treatment for lipedema. 

Many patients with lipedema also have overweight or obesity. In those patients, medications such as semaglutide or tirzepatide may be an important part of a comprehensive treatment strategy.

GLP-1 treatment may help reduce generalized body fat and improve overall health and surgical candidacy. It can also help distinguish the portion of a patient’s body shape related to generalized obesity from the disproportion caused by lipedema.

After significant weight loss, some patients find that the difference becomes even more apparent: their torso becomes smaller while the legs, hips or arms remain disproportionately enlarged.

That’s where lipedema reduction surgery may become particularly helpful.

Lipedema Reduction Surgery

Unlike weight-loss medications, diet or exercise, lipedema reduction surgery directly removes abnormal fatty tissue from the affected areas.

The procedure typically uses specialized liposuction techniques designed to reduce large areas of disproportionate subcutaneous tissue while taking care to minimize trauma to surrounding structures.

Depending on the extent of lipedema, treatment may involve the:

Who Is a Candidate for Lipedema Reduction Surgery?

Not everyone with large legs has lipedema. Other causes of leg enlargement or swelling should be considered, which is why an individualized medical and surgical evaluation is important.

Do I Need to Lose Weight Before Lipedema Surgery?

Not necessarily, but achieving a healthier and relatively stable weight can improve both surgical planning and overall health.

If generalized obesity is contributing significantly to your body size, Dr. Kaplan may recommend addressing that component first.
