In honor of April 20th, or 420, a sort of national holiday for marijuana, several patients are asking: if you can’t have cigarettes before or after surgery, is it ok to have marijuana before surgery? Most patients know about the issues with cigarettes before and after surgery but is marijuana ok? I’m not going to encourage marijuana use before surgery but here are some important points to consider.

What’s the diff between marijuana and cigarettes in regards to surgery?

Most people think surgeons don’t want patients smoking cigarettes because of the smoke. While the smoke and other things in cigarettes can cause lung cancer, it’s the nicotine in cigarettes (and dip, and nicotine gum and nicotine patches) that doctors are worried about. The nicotine causes blood vessels to constrict or narrow which can reduce blood flow to your incision. Your incision needs good blood flow to heal so if the vessels are constricted and not delivering blood to your incision, your incision has trouble healing. So that’s why we want you to avoid cigarettes and other nicotine products preop.

On the other hand, marijuana has no nicotine. So it won’t affect your wound healing but there is still the issue with smoke. The obvious next question is, if I shouldn’t smoke marijuana, how about ingesting it. Well yes, that’s a great way to avoid the smoke. But keep in mind you probably should avoid being stoned right before your surgery because you need to listen to the doctor’s preop instructions. You need to sign consent forms. So it’s best you’re in a sound state of mind to do those things.

Have a great 420 day and enjoy the video below:

Since the surgery is elective, I can delay their surgery in case they continue to smoke.

Check smokers’ compliance

There are urine and blood tests that can screen for the by-products of nicotine to ensure compliance.

While patients think they’re supposed to quit smoking due to the risks of lung cancer, which is true, the main problem when it comes to surgery is not smoking, it’s the nicotine.

So I’m clear with the patient that not only do they need to quit smoking, they need to quit all forms of nicotine (patches, gum, dip, cigs, cigars) since the nicotine impedes the normal ability to heal incisions. Incisions that don’t heal well can trap bacteria and lead to infection.

If a patient states they can only stop temporarily, I require they quit 2 weeks before and 2 weeks after surgery.

Sure it’s possible for a patient to lie and “fool” me and get their surgery. But in the end, they’re only fooling themselves if they risk an infection for an operation they just paid out of pocket for.
